Galiri Care Support Services Pty Ltd (“Galiri Care Support”, “we”, “us”) is committed to protecting your privacy. This policy explains how we collect, use, disclose and protect personal information, and how you can access, correct or complain about our handling of it. We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

The information we collect

What we collect depends on your relationship with us. It may include:

  • Contact details — name, address, phone number and email address.
  • NDIS information — your NDIS participant number, plan details, funding category and plan dates.
  • Health and support information — your support needs, disability-related information, medical or allied health information relevant to delivering your supports, and any risks we need to manage to keep you safe. This is sensitive information and attracts additional protection under the Privacy Act.
  • Details of the people who support you — family members, guardians, nominees, support coordinators and emergency contacts.
  • Service records — rosters, progress notes, incident records and invoicing information.
  • Job applicant information — if you apply to work with us, your CV, qualifications, work rights and screening check results.
  • Website information — information you submit through our contact forms, and standard technical data such as your IP address and browser type.

How we collect it

Wherever it is reasonable and practical, we collect personal information directly from you — for example when you contact us, complete a form on this website, or take part in planning your supports. Sometimes we collect it from others, such as your support coordinator, plan manager, family member, guardian, the National Disability Insurance Agency, or a health professional involved in your care. We do this only where you have consented or where the law permits.

Why we collect and use it

We collect personal information so we can:

  • assess whether we can meet your support needs, and plan and deliver those supports;
  • match you with suitable support workers and manage rostering;
  • keep you and our workers safe, and meet our duty of care;
  • process invoices and claims, including through the NDIS, a plan manager or you directly;
  • respond to your enquiries, feedback and complaints;
  • meet our obligations under the National Disability Insurance Scheme Act 2013 and the NDIS Practice Standards, and to other regulators; and
  • recruit and manage staff.

We do not sell your personal information, and we do not use it for marketing without your consent. Where we do send you updates, you can opt out at any time.

Who we disclose it to

We may disclose your personal information to:

  • our support workers and staff, so far as they need it to deliver your supports;
  • people you have authorised, such as family members, guardians, nominees or advocates;
  • your support coordinator, plan manager or the National Disability Insurance Agency, for the purposes of your plan;
  • health and allied health professionals involved in your care, with your consent;
  • our service providers, such as IT, rostering and accounting providers, who are bound to protect it;
  • the NDIS Quality and Safeguards Commission, or other authorities, where required or authorised by law — including where there is a serious risk to a person’s life, health or safety.

We do not routinely disclose personal information overseas. Where a service provider we use stores data outside Australia, we take reasonable steps to ensure it remains protected to Australian standards.

How we protect it

We take reasonable steps to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. These include access controls, secure storage of physical and electronic records, confidentiality obligations for our staff, and staff training on privacy and information handling. We keep personal information for as long as we need it to deliver supports and to meet our legal and record-keeping obligations, and then destroy or de-identify it.

Accessing and correcting your information

You can ask us for a copy of the personal information we hold about you, and you can ask us to correct it if it is inaccurate, out of date or incomplete. Contact us using the details below and we will respond within a reasonable period. In most cases there is no charge. If we cannot give you access, we will explain why in writing.

Making a complaint

If you believe we have mishandled your personal information, please contact us first using the details below so we can try to put it right. We take complaints seriously, and making one will never affect the supports you receive.
